How to use your Patient Portal for online paperwork:

Web Portal  Welcome! 


*This only works if you have initiated contact, and have a user name and password. 


1) Go to https://bruhn.carepaths.com  


2) Click on "Patient" under "Who are you?"  *right hand corner of the page*


3) Enter your user name and password
---user name is your first and last initial and the last 4 digits of your SSN, then below that
---your password which is your date of birth in 6 digit form, i.e.:  100160 if your date of birth is Oct. 1st, 1960.  Leave off the 19 in front of the year, and add a 0 if your month or day of birth is single digit.


4) Click "Sign in"

My To Do List---------See which documents need to be completed now.


Messages-------------This allows you to send a confidential message to your therapist.

5) Important Reminder: Once you click on "My To Do List" to fill out the online paperwork, you finish each document by following the directions at the bottom of the page, some of these documents require your password  (DOB in 6 digit form) and then you click "Finished:  Sign and Complete" others do not require a password, but you must still click "Finished:  Save and Complete".   If you do not click the Save or Sign and Complete button, all your hard work will not be saved.

Crisis Management

Due to the nature of this work I realize that suicidal ideation can be an issue. It is my priority to help you stay safe. I will do everything in my power to help you maintain safety on an outpatient basis. Sometimes this is not possible, and we will decide together if you need a more intensive level of care. If this is necessary, we can work with your insurance company to gain access to a behavioral health hospital.  If you are feeling unable to resist plans of harming yourself or others call 911 or go to the closest emergency room.  From there, please contact me .


If you are having an emergency you can also call 911 and request mobile crisis. Or call 1-800-SUICIDE (1-800-784-2433) or 1-800-273-TALK (1-800-273-8255).

How to obtain benefits information and prior authorization

 

David Michael Bruhn, LPC participates with most insurance companies. However, it would be a good idea to check with your insurance company for in-network benefits prior to your appointment with this office. If your insurance company requires an authorization for service, please make sure we have this authorization to cover your visit/visits with this provider. As a courtesy, we will be happy to file your medical insurance for you. However, in the event services are non-covered, you will be responsible for the bill. We cannot bill your insurance company without the proper information, so please make sure all your insurance information is accurate and up to date, including your address and phone number, and insurance information. Please be advised that all co-pays, coinsurance and deductibles are due at the time of service. For your convenience, we accept cash, checks, MasterCard and Visa payments.  

 

If prior authorization is required.

  1. Call the number listed on the back of your primary insurance card for “Behavioral or Mental Health and                                     Substance Abuse Benefits” (or call the main number if there is no separate number for Mental Health).  

  2. From the voice-mail menu, choose the “Check Benefits” option

  • 3. Verify that David Michael Bruhn, LPC is in network.

    4. Give the customer service representative your insurance ID number, and ask them: Do I need a “Preauthorization” for this Behavioral Health service code: 90791,90834, 90839, or 90837  please obtain:    
         a) Authorization Number
         b) start and end dates of this Authorization
         c) How many sessions have been authorized today? 
         d) What is my deductible
         e) How much of it have I met?
         f) If in deductible, what is the contractual agreement for this service?
         g) What is my co-pay or co-insurance amount for each session?
         h) What is the Electronic Payer I.D. for Behavior Health Claims?
          i) Do I have any pre existing condition clause that might interfere with payment?
